Physical Therapist Assistant / PTA – Part-time

Continuum Therapy Partners is hiring a part-time Physical Therapist Assistant to join the rehabilitation team at Jerseyville Nursing & Rehab Center in Jersey, Jasper County, Illinois. About the Role

As a Physical Therapist Assistant at Jerseyville Nursing & Rehab Center, you'll provide direct patient care under the supervision of a licensed Physical Therapist. This part-time position offers meaningful work in a supportive facility where your clinical skills directly improve residents' quality of life and recovery outcomes.

What You'll Do

• Execute physical therapy treatment plans, including therapeutic exercises, manual therapy, and gait training tailored to each resident's goals
• Document patient progress, functional improvements, and treatment responses in medical records
• Educate residents and families on exercises, adaptive techniques, and fall prevention strategies
• Assist residents with transfers, mobility aids, and activities of daily living as part of therapy sessions
• Collaborate daily with physical therapists, nursing staff, and the interdisciplinary care team
• Maintain treatment equipment and ensure safe, clean therapy spaces
• Support discharge planning by providing recommendations for home safety and continued care

What We're Looking For

• Current Physical Therapist Assistant license (PTA) issued by the state of Illinois
• Ability to work part-time hours with flexibility around patient census and facility scheduling needs
• Strong clinical skills and knowledge of therapeutic modalities and rehabilitation principles
• Excellent communication and patient care abilities
• Physical capability to assist residents with mobility and transfers safely
• Commitment to working as part of an interdisciplinary team in a skilled nursing setting
