What You'll Do

• Conduct comprehensive psychosocial assessments and develop individualized care plans tailored to client needs
• Provide crisis intervention and supportive counseling for individuals and families experiencing acute challenges
• Coordinate resources and connect clients with community services, benefits, and healthcare referrals
• Advocate for client rights and welfare within the healthcare system and community agencies
• Maintain detailed, compliant documentation of client interactions, treatment progress, and outcomes
• Collaborate with interdisciplinary healthcare teams to support integrated patient care delivery
• Facilitate support groups or educational sessions addressing social determinants of health

What We're Looking For

• Current, unrestricted Licensed Social Worker (LSW) license valid in California
• Minimum two years of social work experience in healthcare, behavioral health, or community services
• Strong assessment and case management capabilities with diverse populations

Position Details
