About the Role

As a Care Coordinator for our OhioRISE program, you'll be the essential link between clients and the behavioral health services they need. Working directly in homes and throughout the Muskingum County community, you'll assess needs, develop individualized care plans, and connect people with resources that support recovery and well-being. This is frontline work with direct, visible impact.

What You'll Do

• Meet with clients in their homes and community settings to understand their needs and build supportive relationships
• Connect clients with behavioral health services, community resources, and social support systems
• Develop and maintain individualized care plans that reflect each person's goals and circumstances
• Monitor client progress and adjust plans as needed to ensure success
• Coordinate with other providers, family members, and community partners to deliver integrated care
• Document interactions, maintain confidential records, and ensure regulatory compliance
• Advocate for clients' needs within systems and remove barriers to their progress

What We're Looking For

• Genuine empathy and commitment to behavioral health and recovery
• Strong communication and interpersonal skills to build trust across diverse populations
• Excellent organizational skills and ability to manage multiple clients and documentation
• Comfort working independently in the field and collaborating with a clinical team
• Valid driver's license and reliable transportation throughout Muskingum County
• Ability to handle sensitive information professionally and maintain strict confidentiality
• High school diploma or equivalent; relevant behavioral health experience is a plus
