Lifeline Community Services is hiring a full-time Case Manager, Transitional Youth Housing in Vista, San Diego County to guide foster and probation youth as they build the skills and confidence to live independently.
About the Role
As a Case Manager supporting our LifeSpring transitional youth housing program, you will work under the guidance of the LifeSpring Program Manager to deliver hands-on, intensive case management. This Vista-based position centers on equipping young people aging out of foster care or exiting the probation system with the practical tools they need to step into adulthood with stability and purpose.
Key Responsibilities
- Provide intensive, individualized case management to foster youth and probation youth preparing for independent living.
- Coach participants in developing essential independent living and life skills.
- Support youth in securing employment and pursuing their educational ambitions.
- Partner with each young person to create personalized life plans that promote lasting self-sufficiency.
- Collaborate closely with the LifeSpring Program Manager and program team to track participant progress.
Qualifications
- A genuine commitment to empowering transition-age youth toward independence.
- Experience or background relevant to case management, social services, or youth-focused programs.
- Strong skills in mentoring, goal-setting, and connecting individuals to community resources.
- Ability to work both independently and as part of a supportive, mission-driven team in Vista, San Diego County.
