AvoCare, in partnership with Centrix Health Resources in Chandler, Arizona, is searching for a compassionate, bilingual (Spanish/English) Medical Social Worker (MSW/BSW) to join our part-time palliative care team and make a meaningful difference in the lives of seriously ill patients across Arizona.

About the Role

This part-time AvoCare Medical Social Worker (MSW/BSW) position in Chandler, Arizona centers on supporting patients living with serious and life-limiting illnesses. As a bilingual Spanish-speaking social worker, you will help individuals and their families navigate emotional, social, and practical challenges while receiving palliative care. The role offers flexible scheduling and adaptable coverage areas, making it well suited for clinicians who value a patient-centered, mission-driven environment. This is an immediate opening.

Key Responsibilities

• Conduct psychosocial assessments for patients and families coping with serious illness.
• Provide emotional support, counseling, and guidance throughout the palliative care journey.
• Connect patients with community resources, services, and support systems.
• Communicate effectively in both Spanish and English to serve a diverse patient population.
• Collaborate with the interdisciplinary palliative care team to enhance quality of life.
• Maintain accurate, timely documentation in accordance with care standards.

Qualifications

• Master's (MSW) or Bachelor's (BSW) degree in Social Work.
• Bilingual fluency in Spanish and English.
• Background or interest in palliative, hospice, or medical social work preferred.
• Strong interpersonal, assessment, and communication skills.
• Ability to work part-time with flexible scheduling across assigned coverage areas in Arizona.
