This full-time position offers the opportunity to empower individuals to achieve stability and wellness through person-centered, community-based care. Responsibilities Provide assertive community treatment services to clients in need of support and intervention. Collaborate with interdisciplinary teams to develop and implement comprehensive care plans for clients. Offer individual and group therapy sessions to address various mental health issues. Monitor client progress and adjust treatment plans as necessary. Ensure adherence to all relevant clinical guidelines, policies, and procedures. Qualifications A master's degree in social work or a related field is required. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) certification is necessary.
