About the Role
As a Temporary Social Worker at Greentree Manor Nursing and Rehabilitation Center, you will help shape and strengthen our Social Services Department, ensuring each resident in Jordan Village, New London County is supported in reaching and sustaining their highest practicable level of wellbeing. This full-time, on-site position centers on identifying and addressing the medically related social needs that arise throughout a resident's stay.
Key Responsibilities
- Contribute to the planning, rollout, and continual review of the Social Services Department's programs and procedures.
- Assess residents to recognize medically related social needs and coordinate appropriate support.
- Help residents attain or maintain their highest practicable physical, mental, and psychosocial wellbeing.
- Support the day-to-day oversight and supervision of social services activities.
- Collaborate with the interdisciplinary care team to advance resident-centered outcomes.
Qualifications
- Background in social work within a nursing, rehabilitation, or long-term care environment.
- Working knowledge of social services delivery and the regulatory standards governing resident care.
- Strong interpersonal, assessment, and documentation skills.
- Availability for a full-time, on-site temporary assignment in Jordan Village, New London County.
