About the Role

As a Social Worker on the Inpatient Case Management team, you will be a vital part of patient-centered care at Sanford Health in Bismarck, North Dakota. This day-shift position partners with multidisciplinary teams to coordinate safe transitions, connect patients with community resources, and advocate for the well-being of those in your care.

Key Responsibilities

• Assess the psychosocial, emotional, and discharge-planning needs of inpatients and their families.
• Develop and carry out individualized care plans in collaboration with physicians, nurses, and care teams.
• Facilitate smooth, safe transitions from the hospital to home, rehabilitation, or other care settings.
• Link patients to community programs, financial assistance, and ongoing support services.
• Document case activity accurately and uphold standards of confidentiality and regulatory compliance.

Qualifications

• Background and credentials in social work appropriate for an inpatient case management role.
• Strong assessment, communication, and care-coordination abilities.
• Capacity to work effectively within a collaborative, fast-paced hospital environment.
• Availability to work an 8-hour day shift, 40 scheduled hours per week, on-site in Bismarck.

Compensation & Details

• Salary range of $25.50 – $36.00.
• $10,000 sign-on bonus.
• Full-time, day shift (8 hours), 40 hours weekly.
• Union-represented position.
