About the Role
As a Self-Directed Service Coordinator with Pathways Of The River Valley in Claremont, Sullivan County, you will collaborate closely with individuals and their support networks to plan, deliver, and oversee personalized services. The position centers on honoring each person's right to direct their own supports and make meaningful decisions about their daily life.
Key Responsibilities
- Partner with individuals and the people who support them to co-design service plans rooted in personal goals and preferences.
- Coordinate and implement supports that strengthen self-determination and decision-making authority.
- Track the delivery of services and review progress to ensure plans continue to reflect each person's evolving needs.
- Promote individual choice across every stage of planning and support.
- Maintain strong working relationships with families, caregivers, and community resources.
Qualifications
- A genuine commitment to advancing the independence and choices of individuals with developmental disabilities.
- Strong collaboration and communication skills for working alongside individuals, families, and support teams.
- Organizational ability to plan, coordinate, and monitor multiple individualized service plans.
- Availability for full-time, on-site work in Claremont, Sullivan County (this role is not remote).
