Position Summary In this role, you'll work closely with HealthSnap's remote monitoring platform to deliver direct patient support. Your primary responsibility involves connecting with patients managing chronic conditions such as diabetes and hypertension, collaborating with their physician teams to drive better health outcomes. You'll manage a caseload of assigned patients throughout each month while remaining available to assist with additional cases as needed. Your success hinges on building meaningful relationships with patients that inspire them to take ownership of their health management. Core Duties Conduct patient consultations via phone with individuals enrolled in chronic condition management programs Deliver education and coaching on lifestyle modifications and evidence-based strategies to support patient adherence to provider-approved care plans Create and sustain comprehensive, current patient documentation following all established protocols Identify and troubleshoot barriers to care adherence while collaborating with providers to find workable solutions Maintain clear, compassionate communication with patients and coordinate with physician teams using established care pathways Contribute to ongoing training initiatives and process improvement efforts that enhance care quality Review and act on health metrics transmitted remotely by patients, including blood pressure, glucose, weight, and oxygen saturation, according to partner protocols Skills & Qualifications Required: Active, current Alaska nursing license (LPN/LVN) in good standing Minimum three years in direct patient care roles, including primary care, specialty practice, home health, or disease management and remote monitoring environments Strong interpersonal and communication capabilities Proven organizational and time-management proficiency Hands-on experience with electronic health record (EHR) systems and care management platforms Capacity to work both autonomously and collaboratively Genuine commitment to patient-centered, empathetic care delivery Dependable broadband internet connection HIPAA-compliant home office or workspace Comfort and competence with videoconferencing and team collaboration tools Preferred: Compact nursing license from additional states HealthSnap covers the cost of obtaining additional state licensures as needed.
