Phoenix Home Care and Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager specializing in hospice care to join our team in St. Louis, Missouri. In this full-time position, you'll combine clinical expertise with care coordination to provide dignified end-of-life support to patients and families navigating one of life's most meaningful transitions. The Opportunity This Registered Nurse (RN) Case Manager Hospice role is central to delivering person-centered, palliative care across our service area. You'll work directly with interdisciplinary teams—physicians, social workers, spiritual care providers, and home health aides—to develop and implement comprehensive care plans that prioritize comfort, autonomy, and family support. Your clinical judgment, compassion, and organizational skills will directly shape the quality of life for our hospice patients and the peace of mind for their loved ones. What You'll Do Conduct thorough initial and ongoing patient assessments, documenting clinical findings and symptom management needs Coordinate and supervise direct patient care services, monitoring medication adherence and therapeutic responses Develop individualized care plans in collaboration with the interdisciplinary hospice team and in alignment with patient goals Educate patients and families about disease progression, comfort measures, and available support resources Manage communications between patients, families, physicians, and care team members to ensure cohesive, responsive care Perform clinical documentation and maintain accurate patient records compliant with regulatory standards Advocate for patient rights, preferences, and dignity throughout the care process Qualifications Current RN licensure in Missouri; BScN or ADN required Demonstrated experience in hospice, palliative care, oncology, geriatrics, or related acute or long-term care settings Strong clinical assessment skills and ability to recognize changes in patient status Excellent verbal and written communication abilities and cultural competency in a diverse community Capacity to work autonomously while collaborating effectively within team structures Valid driver's license and reliable transportation for in-home patient visits About the Company Phoenix Home Care and Hospice provides compassionate end-of-life care, palliative services, and supportive care coordination throughout the St. Louis region.
