Responsibilities Conduct comprehensive clinical assessments—initial evaluations, periodic check-ups, and urgent visits—to guide continuous participant care and monitoring Develop individualized care plans in close partnership with physicians and the interdisciplinary care team Coordinate care transitions, manage placements in skilled nursing facilities, and arrange discharges back to home-based settings Make regular in-person visits to participants in their homes and residential settings based on clinical need Analyze health outcomes and utilization data to support evidence-based practice improvement and quality initiatives What You Bring Active RN license and current Nurse Practitioner or Physician Assistant certification with prescriptive authority Minimum two years of direct clinical experience with frail, elderly, or geriatric populations Demonstrated knowledge of chronic disease management and geriatric best practices Valid driver's license and reliable personal transportation Willingness to share rotating on-call responsibilities, including nights and weekends Completion of an accredited nursing degree program About the Company WelbeHealth operates PACE programs that empower seniors to maintain dignity and independence.
