In this full-time role, you'll deliver direct patient care, develop treatment plans, and help our clients achieve their mobility and independence goals in the comfort of their own homes. Role Overview As a Physical Therapist with our organization, you'll work with patients across a range of conditions, ages, and care settings within the home and community environment. This is an opportunity to make a direct, meaningful impact on patient outcomes while practicing the skilled rehabilitation and therapeutic expertise you've trained for. You'll collaborate closely with interdisciplinary care teams—physicians, nurses, aides, and social workers—to ensure comprehensive, coordinated care aligned with each patient's clinical goals and personal values. Responsibilities Evaluate patients in their homes; assess mobility, strength, balance, pain, and functional limitations Design individualized physical therapy plans of care based on medical diagnosis, patient goals, and evidence-based practice Implement therapeutic interventions, including therapeutic exercise, manual therapy, gait training, and fall prevention strategies Monitor patient progress; adjust treatment plans based on clinical response and changing needs Document patient encounters thoroughly and accurately in compliance with regulatory standards Communicate findings and recommendations to the care team and referring physician Educate patients and caregivers on exercises, safety, and strategies to maintain or improve functional independence Requirements Current, valid Physical Therapy license in Missouri Doctor of Physical Therapy (DPT) or equivalent credential from an accredited program Ability to travel to patients' homes and work flexibly across the Lebanon, Missouri area Excellent communication and interpersonal skills; compassion for patients and families Strong clinical reasoning and problem-solving abilities Professional demeanor and commitment to patient privacy and ethical practice More About Phoenix Home Care and Hospice Phoenix Home Care and Hospice is committed to providing compassionate, high-quality care to patients and families during life's most important moments.
