About the Role
As a Parent Support Advocate (4985) based in Deerfield, Oneida County, you will serve as a vital connection between our care team and the families we serve. This full-time, on-site position centers on guiding caregivers through the support journey of youth enrolled in Thrive Together — a short-term transitional program for young people ages 11 to 17 who are medically cleared for discharge yet require extra preparation before going home or rejoining their community.
Key Responsibilities
- Deliver hands-on outreach, guidance, and encouragement to the parents and caregivers of youth receiving services.
- Champion the needs of families, ensuring their voices are heard throughout the care and transition process.
- Build trusting relationships that help caregivers feel prepared to welcome youth back home or into the wider community.
- Connect families with helpful resources and ongoing support during this short-term transitional period.
- Collaborate with program staff to promote positive, stable outcomes for each young person and their household.
Qualifications
- A genuine passion for advocating on behalf of youth and their families.
- Strong communication and relationship-building skills with caregivers from diverse backgrounds.
- The ability to offer patient, empathetic support during sensitive transitions.
- Availability to work full-time, on-site in Deerfield, Oneida County.
