About the Role

As an LPN / LVN at this Nassau-area rehabilitation hospital, you will work alongside a multidisciplinary team — registered nurses, physical and occupational therapists, physicians, and care coordinators — to support patients on their road to recovery. What You'll Do

• Administer prescribed medications and treatments, accurately documenting all interventions in the patient record
• Monitor and record vital signs, functional status, and changes in patient condition, escalating concerns to the supervising RN or physician promptly
• Assist patients with activities of daily living while supporting individualized rehabilitation goals
• Perform wound care, catheter care, and other skilled nursing procedures within your scope of practice
• Participate in interdisciplinary care conferences, contributing nursing observations to the broader treatment plan
• Provide patient and family education on medications, home care techniques, and discharge instructions
• Maintain a safe, clean care environment in compliance with facility and regulatory standards

What We're Looking For

• Current, unencumbered LPN or LVN license valid in the state of practice
• Active BLS / CPR certification
• Prior experience in a rehabilitation, long-term acute care, or skilled nursing setting is a plus
• Ability to prioritize and manage multiple patients in a fast-moving rehab environment
• Strong documentation habits and attention to clinical detail
• Collaborative mindset with clear, professional communication skills
• Commitment to delivering compassionate, patient-centered care
