Theoria Medical seeks a temporary, full-time Provider Liaison Medical Assistant to join our Lima, Ohio team. Alongside our physician partners and clinical teams, you'll strengthen essential connections between residents and their healthcare providers by managing appointments, supporting virtual consultations, and ensuring residents receive personalized, coordinated care. Your role uniquely blends administrative coordination with direct patient interaction, requiring both operational precision and genuine compassion. Key Responsibilities Schedule and facilitate telemedicine visits and provider appointments including routine, follow-up, and acute-care encounters Prepare residents for provider visits and maintain detailed electronic health records with comprehensive patient histories Coordinate transitions of care from admission through discharge, including case management, authorizations, and connection to follow-up services Educate residents and families on medication adherence, nutrition, fall prevention, and wellness topics based on provider guidance Partner with licensed nursing staff to escalate clinical concerns to providers when necessary Manage prior authorizations, program enrollment forms, and coordination of ancillary services including home health, medical equipment, and rehabilitation therapy Attend interdisciplinary team meetings and provide clinical information to support resident care planning Build relationships with hospital and facility leadership to enhance program visibility and facilitate appropriate patient referrals Who We're Looking For Completion of an accredited Medical Assistant program Certified Medical Assistant (CMA) credential strongly preferred Hands-on experience as a medical assistant or in care coordination, health plan liaison, or similar healthcare roles Demonstrated proficiency with electronic medical record systems Intermediate computer skills including spreadsheets, email, and web-based applications Strong verbal and written communication ability Working knowledge of geriatric healthcare needs and practices Physical ability to independently lift up to 20 pounds and assist with heavier resident transfers as part of a team Capability to travel…
