Housing Stability Case Manager — YWCA of Greater Harrisburg — Penbrook, Dauphin County The Opportunity The YWCA Greater Harrisburg is seeking a passionate and dedicated full-time Transitional / Rapid Rehousing Case Manager to join our team. This position works directly with individuals and families experiencing homelessness to complete intakes, develop individualized service plans, connect them to community resources, and guide them through the process of securing and maintaining safe, affordable housing. Your Responsibilities Conduct comprehensive intake assessments for clients seeking assistance Create personalized service plans tailored to each client's needs Provide direct support in accessing community resources such as housing, financial assistance, and legal aid Assist clients with securing safe, affordable housing through various programs and initiatives Monitor progress and adjust service plans as necessary to ensure long-term stability Collaborate with other case managers, social workers, and community partners to provide holistic support Requirements Bachelor's degree in Social Work, Human Services, or a related field Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred but not required Experience working with individuals and families experiencing homelessness Strong interpersonal and communication skills to build trust and rapport with clients Proficiency in Microsoft Office Suite, especially Excel for data management Prioritization of diversity, equity, and inclusion in all work practices Working at YWCA of Greater Harrisburg At the YWCA Greater Harrisburg, we are committed to creating a supportive and inclusive environment where every individual can thrive.
