As a Community Partners Home Based Case Manager, you will deliver supportive services directly in the homes of families across Lafayette and Tippecanoe County. This is a relationship-centered, full-time position rooted in Bauer Family Resources' commitment to empowering children and their families to thrive. You will be a steady, caring presence, helping households connect with the resources and guidance they need to grow.

Key Responsibilities

• Provide home-based case management services to children and families throughout the community.
• Assess each family's strengths, needs, and goals, then develop individualized plans to support them.
• Connect families with community partners, programs, and resources that promote stability and well-being.
• Maintain accurate, timely documentation and case records in line with program expectations.
• Coordinate with internal teams and external agencies to ensure consistent, wraparound support.
