Community Care Program Consultant

Role Overview

The Community Care Program Consultant will support the growth and development of PurposeCare‘s Illinois CCP service line through a 90-day consulting engagement. The ideal consultant will bring deep knowledge of the Illinois CCP landscape and strong operational and relationship-building experience.

What You Will Do

Conduct a comprehensive assessment of PurposeCare‘s current CCP operations, evaluate long-term growth opportunities, and develop a strategic playbook outlining recommendations for CCP growth strategy, community partnership development, and operational best practices.

Requirements

• Extensive knowledge of the Illinois Community Care Program
• Experience working within home care, healthcare operations, community outreach, or healthcare growth strategy
• Strong understanding of CCU relationships, referral development, and Medicaid/community-based programs
• Proven experience building, scaling, or optimizing operational programs
• Strong analytical, organizational, and strategic planning skills
• Ability to work collaboratively with executive and operational leadership teams
• Exceptional communication and relationship-building abilities
