About the Role As a Clinical Case Manager with PATH's Valley Vista Permanent Supportive Housing program, you'll provide direct support to HUD VASH participants—primarily veterans transitioning from homelessness to stable housing. This role combines clinical expertise with hands-on field work, where your interventions help clients build the skills and connections necessary for independent living and lasting recovery. What You'll Do Deliver mobile and in-community case management across housing stabilization, financial literacy, employment preparation, and community resource linkage Develop individualized Housing Stability Plans that address both immediate and long-term client goals Coordinate with VA partners and local health and behavioral health providers to connect clients with substance abuse treatment, mental health, and primary care services Apply evidence-based practices including Critical Time Intervention, Harm Reduction, Motivational Interviewing, and Housing First methodologies Monitor progress toward treatment and housing goals, adjusting plans as clients' circumstances evolve Support clients in problem-solving, skill-building, and self-directed care management Maintain thorough, timely documentation and collaborate with multidisciplinary VASH and VA teams What We're Looking For Master's degree in a clinical or social service field (MSW, MFT, clinical counseling, psychology, or equivalent) Minimum one year of professional experience in healthcare, case management, homelessness services, or work with vulnerable populations Demonstrated proficiency with evidence-based clinical interventions and case management frameworks Intermediate computer skills including Microsoft Office and database systems Demonstrated ability to engage clients with dignity and without judgment despite complex, multi-faceted needs Strong self-motivation and capacity to prioritize independently in a dynamic environment Flexibility to work variable schedules, including evenings and weekends as needed Valid California driver's license, reliable personal transportation, and proof of insurance Willingness to obtain or maintain CPR and First Aid certification Full COVID-19 vaccination status Preferred Qualifications Active California Board of Behavioral Sciences licensure (ACSW, LCSW) About PATH PATH is a mission-driven organization committed…
