Case Worker Outreach

GHI Gateway Healthcare is hiring a full-time Case Worker Outreach professional in Pawtucket, Providence County, to deliver compassionate, community-based support to adults living with serious, long-term mental illness.

About the Role

As a Case Worker Outreach team member with GHI Gateway Healthcare in Pawtucket, you will work under the direction of a Program Manager and/or Team Leader to bring case management, rehabilitation support, and crisis response directly to the people who need it. This is hands-on, relationship-driven work carried out across home, office, and community settings throughout Providence County.

Key Responsibilities

• Deliver case management and rehabilitation services to adults coping with serious, long-term mental illness.
• Provide outreach and crisis intervention wherever clients are — in their homes, at the office, or out in the community.
• Partner with psychiatric and allied mental health colleagues on assessment, diagnosis, and intervention.
• Contribute to individualized treatment planning as part of a coordinated care team.
• Maintain ongoing communication with your Program Manager and/or Team Leader to ensure client needs are met.

Qualifications

• Experience supporting adults with serious or persistent mental health conditions.
• Comfort working independently in home, office, and community environments across the Pawtucket area.
• Strong collaborative skills for working alongside psychiatric and mental health practitioners.
• Ability to respond calmly and effectively during crisis situations.
• A commitment to client-centered, recovery-oriented care.
