Case Planner

MercyFirst is hiring a full-time Case Planner in Brooklyn, New York City to support vulnerable children and families through our Family Support Services team. This hybrid, on-site role offers a meaningful opportunity to build stability and brighter futures across the community.

About the Role

As a Case Planner at MercyFirst, you will partner directly with families to coordinate services, develop individualized plans, and guide them toward lasting positive change. Reporting to a Casework Supervisor within the Family Support Services department, you will serve as a steady advocate for the children and parents in your care. This full-time position is based in Brooklyn, NY and follows a hybrid schedule, and it currently includes a $1,000 hiring incentive.

Key Responsibilities

• Develop, monitor, and update tailored case plans that reflect each family's strengths and needs.
• Conduct regular visits and check-ins to assess progress, safety, and overall well-being.
• Connect families with community resources, support programs, and essential services.
• Maintain accurate, timely documentation and case records in line with agency and regulatory standards.
• Collaborate closely with your Casework Supervisor and the broader Family Support Services team.
• Advocate for the best interests of children and families across schools, courts, and partner agencies.

Qualifications

• A genuine commitment to serving children and families in need within a mission-driven, not-for-profit environment.
• Strong organizational, written, and interpersonal communication abilities.
• Capacity to manage a caseload, prioritize effectively, and respond with empathy and sound judgment.
• Ability to work on-site in Brooklyn under a hybrid arrangement and collaborate with a dedicated casework team.
