Case Manager – Ulster

About the Role

As a Case Manager in our Children & Families Support Initiative (CFSI), you will deliver community-based services to young people and their families in Ulster County. Operating with intentionally manageable caseloads, you will offer strength-based, well-informed guidance that helps youth navigate challenges and build on their abilities. This is a hands-on, relationship-driven position rooted in collaboration across the local community.

Key Responsibilities

• Provide direct, community-based case management to youth and their families through the CFSI program
• Maintain a balanced caseload to ensure each family receives focused, individualized attention
• Coordinate closely with schools, courts, and community partners to support coordinated care
• Deliver strength-based, trauma-informed support that empowers youth and reinforces family stability
• Document progress and tailor service plans to the evolving needs of each household

Qualifications

• A genuine commitment to working alongside youth and families in a community setting
• Comfort partnering with schools, the courts, and other community-based organizations
• Strong interpersonal, organizational, and communication skills
• Ability to apply a strength-based, person-centered approach to casework
• Availability for a full-time, on-site schedule based in Hurley, Ulster County (this role is not remote)
