The Opportunity As a Case Manager - RENEW FSP with Bayareacs in Sacramento, California, United States, you will deliver high-quality, person-centered interventions outside traditional office walls. The position requires travel throughout Sacramento County to engage people who are chronically homeless. Full-time schedules run 8:30 a.m. to 5:00 p.m. on either a Tuesday–Saturday or Sunday–Thursday pattern. You will practice a strength- and needs-based approach that includes skills building, benefits navigation, housing support, crisis response, and family involvement, always oriented toward recovery goals the client owns. What You'll Do Build and deliver a full range of services—individual, family, and group counseling; individual rehabilitation; and case management—for every person on your caseload. Apply evidence-based practices suited to the RENEW FSP population so outcomes stay strong and measurable. Continuously evaluate risk related to symptoms, substance use, living environment, and physical health, then intervene at the right intensity. Determine ongoing service need and co-create person-centered plans that help each individual improve health and meet personally meaningful goals. Maintain active outreach and progressive engagement; alert the team at once when someone disengages and partner on decisions to reduce or end services according to program guidelines. Complete domain-based assessments, referral screening, service and community planning, and benefits coordination that reinforce wellness and recovery; practice therapeutic case management with a “whatever it takes” mindset. Document all activity accurately so charts and reports meet Bayareacs and county standards, and produce the required monthly units of billable community-based service (at least 65 percent of time in face-to-face or other eligible support).
