About the Role As a Case Manager with PATH's Interim Housing Services team, you'll serve individuals across all phases of their journey—from street outreach through interim housing to permanent housing placement. This is direct, hands-on work where you'll support individuals experiencing homelessness toward housing and stability. Driving is an essential part of this position. What You'll Do Conduct intake assessments and work with clients to develop personalized Individualized Service Plans (ISP) addressing housing, income, health, mental health, substance use, financial resources, vocational training, and social support goals Connect clients to community resources, referrals, and support services aligned with their ISP goals Administer standardized assessments such as the VI-SPDAT to evaluate client needs and placement eligibility Complete 90-day progress reviews to evaluate goal progress and adjust support strategies Maintain thorough, confidential case documentation in compliance with HIPAA and PATH policies, ensuring accurate and current client files What We're Looking For You bring strong communication skills, both verbal and written, plus the ability to think critically and solve problems creatively. You collaborate effectively with colleagues, participants, and community partners while maintaining professional boundaries and high ethical standards. Attention to detail, initiative, and a proactive approach to work are essential. Preferred: Two years of experience in healthcare, case management, homeless services, or work with vulnerable populations. Minimum requirements: Employment authorization in the U.S., ability to obtain or current CPR/First Aid certification, valid driver's license with proof of insurance and vehicle ownership, reliable transportation, and ability to pass a tuberculosis test, background screening, and drug screening.
