Case Manager, Independent Living Program

Side By Side is hiring a full-time Case Manager for our Independent Living Program in Philatelic, Alameda County, to walk alongside transition-age youth as they build the skills and confidence to thrive on their own.

About the Role

As the Case Manager for the Independent Living Program, you will serve as a steady, trusted guide for some of the most vulnerable young people in Alameda County. This on-site role centers on helping youth move toward stable, self-directed adult lives, coordinating the supports and resources they need at each step of their journey.

Key Responsibilities

• Provide direct case management to youth enrolled in the Independent Living Program, building trusting, goal-focused relationships.
• Develop individualized support plans that strengthen life skills such as budgeting, housing readiness, education, and employment.
• Connect participants with community resources, services, and referrals across the Bay Area.
• Track each youth's progress, document services accurately, and maintain timely records.
• Partner with internal teams, families, and external agencies to coordinate consistent, wraparound care.
• Advocate for the needs, rights, and well-being of the young people you serve.

Qualifications

• A genuine commitment to supporting vulnerable youth and advancing equitable outcomes in child welfare.
• Experience in case management, social services, youth development, or a closely related field.
• Strong relationship-building, communication, and organizational skills.
• Ability to work on-site in Philatelic, Alameda County, as part of a collaborative team.
• Comfort coordinating across multiple partners and managing documentation with care and accuracy.
