PATH (People Assisting the Homeless) is hiring a full-time Case Manager to join the Interim Housing Services team at our Evans Lane office in Santa Clara County (Position JR 5726). You'll support individuals experiencing homelessness on their path toward stable housing and independence. About the Role You'll provide direct case management services to clients at various stages of homelessness—from street outreach through interim and permanent housing. You'll develop personalized service plans, connect clients with community resources, and guide them toward housing stability and self-sufficiency. This position requires reliable transportation and a valid driver's license for in-person client work. Work Schedule: Tuesday–Saturday, 12:30 PM–9:00 PM or Sunday–Thursday, 7:00 AM–3:30 PM. Salary: $27.75–$33.83/hour, paid over 24 annual pay periods. What You'll Do Conduct intake assessments and evaluations (including VI-SPDAT) to identify client strengths, barriers, and service needs Develop individualized service plans addressing housing, employment, healthcare, mental health, substance use, finances, and family connections Connect clients with local agencies, benefits programs, medical providers, and community resources Complete 90-day progress reviews and adjust plans based on client outcomes and achievement Provide crisis intervention and teach practical coping strategies that build client independence and problem-solving ability Maintain thorough, confidential case documentation in compliance with HIPAA and organizational policies Practice motivational interviewing and trauma-informed care in all client interactions Build collaborative relationships with colleagues, clients, and community partners to improve engagement and outcomes What We're Looking For Required: Authorization to work for any US employer (no visa sponsorship available) Valid driver's license with reliable transportation and vehicle insurance Ability to qualify for PATH's commercial auto insurance policy CPR/First Aid certification or willingness to obtain it Ability to pass background check, tuberculosis screening, and drug test Preferred: Two or more years of professional experience in case management, homeless services, healthcare, mental health support, substance use services, or direct work with vulnerable populations.
