As a Case Manager, you will play a crucial role in supporting clients at risk of or diagnosed with mental illness, as well as their caregivers. Your responsibilities include engaging and collaborating with various stakeholders, providing emotional support through home visits, and ensuring seamless service linkages. What You'll Do Engage and collaborate with stakeholders such as healthcare providers, social workers, and community partners to ensure comprehensive care for clients. Conduct regular home visits to assess the mental health conditions of clients and provide emotional support. Develop and maintain detailed case management plans that address the specific needs of each client. Provide guidance and resources to caregivers to enhance their ability to support clients with mental illness. Facilitate access to appropriate services and resources for clients, ensuring they receive necessary care and support. Qualifications A degree in Social Work or a related field is required. Experience working with individuals with mental health issues and their caregivers is preferred.
