Case Manager

Samaritan Daytop Village is seeking a dedicated full-time Case Manager to join our Rochdale Village, Queens team and help individuals and families on their path from homelessness toward lasting independence.

About the Role

As a Case Manager at Samaritan Daytop Village, you will be the primary point of support for clients enrolled in our transitional housing programs serving single adults and families across New York City's five boroughs. Based in Rochdale Village, Queens, this on-site role focuses on guiding clients through assessment, planning, and connection to the services they need to rebuild stability in their lives.

Key Responsibilities

• Meet one-on-one with clients to conduct intake interviews and build trusting working relationships.
• Assess each client's circumstances, strengths, and immediate needs.
• Partner with clients to create and update personalized Independent Living Plans.
• Locate and connect clients with relevant community resources and support services.
• Coordinate referrals and follow up to ensure clients access the help they require.
• Maintain accurate, timely case records consistent with program standards.

Qualifications

• A genuine commitment to supporting people experiencing homelessness.
• Strong interpersonal, interviewing, and active-listening skills.
• Ability to assess needs and develop practical, client-centered service plans.
• Familiarity with community resources and the referral process.
• Organized, dependable, and able to manage documentation accurately.
• Comfortable working on-site in Rochdale Village, Queens, in a full-time capacity.
