BronxWorks is hiring a full-time Case Manager (Req. 5038) to join our team at the Hub in the Bronx, supporting individuals and families on their path to greater stability and opportunity. About the Role
As a Case Manager at BronxWorks, you will work directly with neighbors of all ages — from young children to older adults — connecting them with the resources, guidance, and advocacy they need to improve their economic and social well-being. This is a full-time, in-person position based in the Hub neighborhood of the Bronx, NY.
Key Responsibilities
- Conduct intake assessments and identify the needs, strengths, and goals of each participant.
- Develop, implement, and regularly review individualized service plans.
- Link clients to community resources such as housing, food assistance, education, and supportive services.
- Provide ongoing case monitoring, follow-up, and advocacy on behalf of participants.
- Maintain accurate, timely, and confidential case records and documentation.
- Collaborate with internal programs and external partners to coordinate comprehensive support.
Qualifications
- Commitment to serving diverse individuals and families with dignity and respect.
- Strong interpersonal, organizational, and communication skills.
- Ability to manage a caseload, meet deadlines, and keep detailed records.
- Comfort working on-site in a community-based setting in the Bronx.
- Background or interest in social work, human services, or a related field.
