About the Role
As a Case Manager at Project Renewal, you will serve as a trusted point of contact for clients in the Bronx, coordinating the services and support they need to move toward health, housing, and employment. This on-site, full-time position is ideal for a compassionate, organized professional who thrives in a collaborative, community-focused environment.
Key Responsibilities
- Assess client needs and develop individualized, goal-oriented service plans.
- Connect clients with internal programs and external resources spanning health care, housing, and job readiness.
- Monitor each client's progress and adjust support plans as circumstances change.
- Maintain accurate, timely case notes and documentation in line with program and regulatory standards.
- Advocate on behalf of clients and coordinate care across multidisciplinary teams.

- Strong interpersonal, communication, and organizational skills.
- Ability to work effectively with diverse populations in a fast-paced setting.
- Comfort maintaining detailed records and managing a caseload responsibly.
