About the Role The Social Worker Discharge Planner plays a vital role in our care coordination team, meeting patients and families at critical transitions. You'll assess individual and family circumstances, identify barriers to safe discharge, and develop individualized plans that connect people with the right services and supports—whether skilled nursing, therapy, in-home care, housing assistance, or financial resources.
