Centurion Health is seeking a part-time Behavioral Health Case Manager to join our team at the DeBerry Special Needs Facility in Belle Meade, Davidson County, Tennessee. As a trusted healthcare provider to the Tennessee Department of Correction, we're looking for a compassionate professional to manage the care coordination and transitional planning needs of individuals within our correctional healthcare system.

About the Role

As a Behavioral Health Case Manager, you'll play a vital role in supporting individuals through assessment, care planning, and the coordination of transition services. This position bridges clinical healthcare and social services within a specialized correctional facility, requiring both clinical knowledge and genuine commitment to supporting individuals through recovery and reintegration planning.

What You'll Do

• Conduct thorough assessments of behavioral health needs and risk factors for individuals in our facility
• Develop and implement comprehensive case management plans tailored to each individual's clinical and social needs
• Coordinate transitional services and release planning, working with community providers and support networks
• Document assessments, treatment progress, and service coordination activities in accordance with healthcare and correctional standards
• Collaborate with clinical, custody, and administrative teams to ensure continuity of care
• Connect individuals with community mental health resources, housing, employment, and social services

What We're Looking For

• Bachelor's degree in social work, behavioral health, counseling, psychology, or a related field
• Strong knowledge of behavioral health conditions and treatment approaches
• Experience with case management, care coordination, or similar roles
• Ability to work effectively with vulnerable populations in structured environments
• Excellent written and verbal communication skills
• Valid driver's license and reliable transportation
