Adult Services Case Manager

Blue Mountain Action Council is hiring a full-time Adult Services Case Manager in Walla Walla, Walla Walla County, to walk alongside community members as they work toward greater stability and self-sufficiency. This is an on-site role for someone who finds purpose in connecting people to the resources that change lives.

About the Role

As an Adult Services Case Manager, you will support adults navigating life's challenges by assessing needs, building individualized plans, and linking clients to the right programs and services. The position is full-time and nonexempt, scheduled for up to 40 hours per week depending on contract funding, and is based entirely in Walla Walla rather than remote.

Key Responsibilities

• Conduct intake assessments and identify each client's goals, barriers, and service needs.
• Develop, monitor, and adjust personalized case plans that promote stability and independence.
• Coordinate referrals to internal programs and partner agencies across the community.
• Maintain accurate, timely, and confidential case records in line with program and contract requirements.
• Advocate for clients while tracking progress and outcomes throughout the service period.

Qualifications

• A genuine commitment to serving adults from diverse and often vulnerable backgrounds.
• Strong organizational skills with the ability to manage documentation and meet reporting expectations.
• Clear written and verbal communication for working with clients and community partners.
• Availability to work on-site in Walla Walla up to 40 hours per week, as funding allows.
