Futures Inc. is seeking a dedicated Individualized Support Provider to join our team in Middletown, Middlesex County. This part-time role offers the opportunity to make a real difference by delivering personalized, one-on-one support to individuals with disabilities building toward greater independence.
About the Role
As an Individualized Support Provider at Futures Inc., you'll be at the heart of our community-based program, offering tailored, direct support to individuals with disabilities. Your work will be centered on fostering personal growth and helping each person develop the skills and confidence they need to live more independently. You'll work one-on-one in a responsive, positive environment designed to bring out the best in every person we serve.
What You'll Do
- Deliver customized, individualized support aligned with each client's personal goals and strengths
- Build authentic, supportive relationships through consistent one-on-one interaction
- Facilitate community-based activities and skill-building opportunities
- Support progress toward independence while adapting approaches to individual needs
- Collaborate with our team to develop and refine personalized growth plans
- Maintain clear communication with supervisors and care coordinators about client progress
- Contribute to a welcoming, empowering program environment
