Transition Coordinator II

Hillside Childrens Center is hiring a full-time Transition Coordinator II in Auburn, Cayuga County, to help youth and families move confidently toward stability and successful discharge from residential care.

About the Role

As a Transition Coordinator II at Hillside Childrens Center in Auburn, Cayuga County, you will provide high-quality psychosocial support to the young people and families served through our Residential Treatment Facility (RTF). Partnering with a multi-disciplinary treatment team and outside care providers, you will help ensure each client's holistic needs are addressed throughout their stay and as they prepare to return to the community.

Key Responsibilities

• Collaborate with the RTF multi-disciplinary team and external providers to coordinate well-rounded care for each youth.
• Work directly with youth and their families on safety planning, discharge planning, and transition readiness.
• Connect clients to community-based resources and support services that promote long-term success after discharge.
• Advocate for the overall well-being of youth and families while monitoring that treatment goals are being met.
• Maintain clear communication and documentation across the care team to support seamless transitions.

Qualifications

• Background in social work, human services, counseling, or a related field suited to psychosocial care delivery.
• Experience supporting youth and families, ideally within a residential or treatment-based setting.
• Strong skills in care coordination, discharge and safety planning, and linking clients to community resources.
• Ability to work effectively as part of a multi-disciplinary team while building trust with youth and families.
• Excellent communication, organization, and advocacy skills.
