Provider Relations

Crossroads Hospice & Palliative Care is hiring a full-time Provider Relations professional based in Euclid, Cuyahoga County — a community-facing role at the heart of connecting patients and families to compassionate end-of-life care.

About the Role

As a Provider Relations specialist, you will serve as the primary point of contact between Crossroads Hospice & Palliative Care and the broader medical community across the greater Cleveland area. Your work directly influences how many patients gain timely access to hospice and palliative services when they need them most.

What You'll Do

• Build and sustain trusted partnerships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare organizations throughout Cuyahoga County
• Educate clinicians and facility staff on hospice eligibility criteria, the referral process, and the full spectrum of services Crossroads provides
• Act as a responsive liaison between referring providers and internal clinical teams to ensure seamless patient transitions into care
• Identify and develop new referral relationships through regular outreach, facility visits, and participation in community health events
• Track outreach activities, referral volumes, and provider engagement to support team goals and inform strategic planning
• Collaborate closely with nurses, social workers, and care coordinators to communicate patient needs and advocate for appropriate, timely admissions
• Represent Crossroads Hospice & Palliative Care professionally at healthcare networking events and community functions in the Euclid and greater Cuyahoga County area

What We're Looking For

• Experience in healthcare sales, business development, provider relations, or a related community-facing healthcare role
• Familiarity with hospice, palliative care, home health, or post-acute care settings is a strong advantage
• Demonstrated ability to cultivate long-term relationships with physicians and healthcare facility staff
• Excellent interpersonal and communication skills — comfortable presenting to clinical audiences and advocating for patients
• Self-directed and organized, with the ability to manage a territory and prioritize outreach independently
• Genuine commitment to improving end-of-life care outcomes for patients and their families
• Valid driver's license and reliable transportation for in-person provider visits throughout Cuyahoga County
