Care Manager

Lifespan of Greater Rochester is seeking a dedicated Care Manager to join the Finger Lakes Caregiver Institute (FLCI) in Brighton, Monroe County. In this part-time, on-site role, you will play a meaningful part in supporting older adults and the families who care for them across our region.

About the Role

As a Care Manager with Lifespan of Greater Rochester, you will connect older adults and caregivers with the resources, guidance, and personalized support they need to navigate the challenges and opportunities that come with a longer life. This position sits within the Finger Lakes Caregiver Institute, a specialized program at our trusted, mission-driven regional nonprofit. The role is classified as non-exempt at 30 hours per week, with an hourly rate of up to $23.00.

Key Responsibilities

• Assess the needs of older adults and their caregivers to shape individualized care plans.
• Coordinate services and link clients to appropriate community resources and support programs.
• Offer ongoing guidance, advocacy, and follow-up so clients receive consistent, high-quality care.
• Partner with colleagues across Lifespan programs and the Finger Lakes Caregiver Institute to deliver integrated support.
• Keep accurate, timely records and documentation in line with program standards.
