About the Role

As a Targeted Case Manager based in Polk County, Florida, you will coordinate outpatient mental health services for adults and adolescents who depend on responsive, compassionate care. This is a 1099 contract position offering a flexible schedule, including morning availability, with pay set at $25.00 per hour. Please note that no benefits are provided under this contract arrangement.

Key Responsibilities

• Deliver targeted case management for adults and adolescents accessing outpatient mental health services.
• Assess each client's needs and link them to the appropriate community and clinical resources.
• Develop, monitor, and update individualized service plans that support recovery and stability.
• Advocate for clients while coordinating with providers, families, and support networks.
• Maintain accurate, timely documentation in line with program and regulatory standards.

Qualifications

• A solid background in mental health services and case management.
• Genuine passion for serving diverse populations of adults and adolescents.
• Strong organizational, communication, and advocacy skills.
• Ability to manage a flexible schedule, including morning hours, as an independent 1099 contractor.
• Commitment to ethical, client-centered practice.
