Social Worker LCSW-C – Per Diem

Genoa Telepsychiatry is seeking a licensed clinical social worker (LCSW-C) for a per diem Social Worker LCSW-C role based in Chestertown, Kent County, Maryland. This is an opportunity to deliver compassionate, person-centered behavioral health and psychosocial support on a flexible, as-needed schedule.

About the Role

As a Social Worker LCSW-C - Per Diem, you will provide clinical social work services to individuals and families across the Chestertown, Kent County community. The position offers schedule flexibility while contributing to a coordinated, whole-person model of care that helps patients and their loved ones feel supported throughout their treatment journey.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized care plans for patients and their families.
• Provide clinical social work support, counseling, and resource navigation in line with each patient's needs.
• Coordinate with physicians, care teams, and community partners to ensure continuity of care.
• Document services accurately and maintain compliance with applicable clinical and regulatory standards.
• Connect patients and caregivers with appropriate community and post-acute support services.

Qualifications

• Active Maryland LCSW-C (Licensed Certified Social Worker-Clinical) credential in good standing.
• Master's degree in Social Work (MSW) from an accredited program.
• Clinical experience assessing and supporting patients and families, ideally in a healthcare or post-acute setting.
• Strong communication, collaboration, and care-coordination skills.
• Availability to work on a per diem, as-needed basis in the Chestertown, Kent County area.
