Outreach & Enrollment Manager

Harbor Health Services, Inc. is hiring a full-time Outreach & Enrollment Manager in South Boston, Suffolk County, to grow access to our Programs of All-Inclusive Care for the Elderly (PACE) and help frail older adults remain safely in the homes and neighborhoods they love.

About the Role

As the Outreach & Enrollment Manager, you will lead the effort to connect eligible seniors and their families with Harbor Health's Elder Service Plan. Through our PACE model, frail elders receive coordinated medical and supportive care that lets them keep living independently in the community. You will champion enrollment growth across our two medical and day centers in Mattapan and Brockton, MA, which together serve more than 580 participants. This is an on-site position based in South Boston.

Key Responsibilities

• Plan and direct community outreach initiatives that build awareness of Harbor Health's Elder Service Plan PACE programs.
• Guide prospective participants and their caregivers through the eligibility and enrollment process from first contact to admission.
• Cultivate referral relationships with hospitals, senior centers, social service agencies, and other community partners.
• Track enrollment goals, monitor outcomes, and report on progress against targets.
• Coordinate closely with clinical and administrative teams at the Mattapan and Brockton centers to support a smooth onboarding experience.

Qualifications

• Demonstrated experience in outreach, enrollment, intake, or community engagement, preferably within healthcare or eldercare services.
• Strong communication and relationship-building skills with diverse seniors, families, and community organizations.
• Ability to manage multiple priorities, meet enrollment goals, and maintain accurate records.
• Commitment to serving frail elders with dignity and compassion.
