As a Direct Support Professional with Take Another Chance, you'll partner directly with the individuals we serve, delivering compassionate, person-centered support that helps each person live as safely, fully, and independently as possible. This is a relationship-driven position based on site in East Cleveland, ideal for a dependable, collaborative professional who takes pride in showing up and making a difference every shift.

Key Responsibilities

• Support individuals with activities of daily living, encouraging skill-building and independence wherever possible
• Administer medication accurately and in line with established care plans and protocols
• Provide safe, reliable transportation to appointments, activities, and community outings
• Promote community integration by helping individuals participate in social, recreational, and everyday experiences
• Champion a person-centered approach that respects each individual's goals, choices, and dignity
• Work closely with team members to maintain a safe, supportive, and welcoming environment
